  • Medicare

  • Cigna

  • United Medicare

  • Aetna

  • MedCost

At this time we currently do no accept Medicaid.

Additional Fees: 

 

There is an additional $25 dollar fee for completion of forms for use outside of the patient's medical record, including:

  • Pre-Employment

  • Employment

  • Life Insurance.

  • Family Medical Leave
